For GPs, clinics and their patients

Not another dashboard. Not another health app. A new lifestyle & prevention ecosystem.

GPs and clinics send evidence-based lifestyle and condition plans. Patients live them in My Lifestyle Hub, with a check-in each day and a 3D twin of themselves that is never graded, and how they’re getting on comes back to the practice for review.

Plans from GPs and clinics, lived every day in My Lifestyle Hub with a 3D twin that’s never graded, and back to the practice for review.

57 plans · free tier with no card · 14-day trial of the rest · Medicare GPCCMP: items 965 and 967

The six pillars

Six pillars, orbiting one person.

Each orb is a pillar of lifestyle medicine, scored from the patient’s own Health Optimisation check: about their habits, never a score of their health. Each day brings one yes-or-no question for each, and the patient chooses which.

Body systems

Look inside: the systems their plans reach.

Eleven body systems, drawn inside the twin as glass. Results sit beside each one exactly as the patient typed them, or as their practice sent them, with the date. The app never compares, flags or grades them.

My Lifestyle Hub

Then the plan goes home with them.

It lands in My Lifestyle Hub, on the web and on their phone: a check-in each day, medals for the habits that hold, and a room of their own.

One ecosystem

Not another dashboard. Not another health app. One ecosystem, joined up.

Most tools stop at the edge of the consultation, or at the patient’s lock screen. GeneralPractice.AI is the whole loop: the clinic prescribes, the patient lives it every day, their twin shows it, and it comes back to the practice for review.

For clinicians · The clinic room

Your clinic is the dashboard.

Sign in and you’re standing in your own clinic, not looking at a page of tables. What needs you and your patients are on the two screens, the plan library and the research are on the shelves, and the post box sends a plan. Every button is the dashboard’s own, and the dashboard itself is one tap away.

Riverside Practice is a sample clinic. Angus, 38, made GeneralPractice.AI; the plans shown with him here are a sample.

A consultation

Your patient beams in, their plans round them.

Connect for a consultation and they step onto the pad as their twin, or as their personalised avatar if they choose; anyone else appears as a standard figure. Each plan hangs beside them, with a line to the part of the body it’s about. Their screen shows only what you put on it: never your dashboard, your notes or another patient.

The main plan, close up

Everything about a plan, in one place.

Open a plan and it comes up close: the factors they scored themselves, the goals they set in their own words, and the check-ins that follow from them. A condition plan waits for your sign-off, which takes a minute or less; a lifestyle plan goes straight to them.

  • Play tennis every Saturday morning
  • Be in bed by 11 on work nights
  • Swap afternoon biscuits for fruit and nuts

Make it yours

Build the clinic you want to work in.

Choose the room’s look, the floor and how much it shines, and the music. On any paid tier, furnish it, hang art, and choose the view outside and the time of day. Each clinician can start on the dashboard instead, and a device that can’t draw 3D gets the same dashboard as a list.

Australia · Medicare

The GPCCMP, prepared from the assessment you already have.

A GP Chronic Condition Management Plan needs conditions, agreed goals, patient actions, likely treatments, the purpose of every referral and arrangements for review — all written down. Your patient has already answered for most of it. GeneralPractice.AI carries that across, marks which words are theirs, and refuses to generate anything until the descriptor is actually met.

The four MBS items

Ruleset

2026-07-01 — MBS Online, note AN.0.47. Item numbers, fees and intervals live in one dated file, so a plan generated today keeps the rules it was made under.

  • IntervalsTwelve months between preparations, three between reviews — and a new plan must also be at least three months after the last review. The dates are worked out for you and shown as guidance, never as a claim.
  • Linked allied healthA plan stays current for linked allied-health access for eighteen months from the last preparation or review. The allied-health provider claims their own item; the GP never does.
  • ReferralsA referral can name any of thirteen allied-health disciplines, and each one has to carry the purpose of the service before the plan will finalise. The plan is not the referral — that stays its own document.
  • Legacy plansA GP Management Plan or Team Care Arrangement from before 1 July 2025 cannot be reviewed under the GPCCMP review items, and the platform says so instead of offering the item.
  • Exceptional circumstancesA significant change in the patient's condition or care can shorten the ordinary intervals — but only with a reason you write yourself, kept with the plan.
  • The recordEvery generated plan is stored as a numbered version with the clinician who made it, and exports to Word for the patient's copy and the medical record.

The item shown is a candidate. Eligibility, the attendance and the claim remain the practitioner's responsibility — the platform prepares the document and checks the descriptor, and you check HPOS before claiming. Included on Solo and Clinical Unlimited.

How it works

Four steps, and only one of them is yours.

The same route whether the patient is sitting with you or answering at home a week before their review.

1Invite

Invite, or start in the room

Send the assessment to one patient or a whole recall list, or generate a link and do it together during the consultation.

You · seconds
2Assess

They answer in their own time

Ten modifiable factors, scored one to ten, in their own words — with the instruments the condition calls for. An evening of theirs, not an appointment of yours.

Patient · about 20 minutes
3Approve

You approve what needs it

Goals, scores, measures and flags on one screen, with the note for the record ready to copy. Condition plans wait for your sign-off; lifestyle and prevention plans go straight out.

4Live it

It becomes something they do

The plan lands in My Lifestyle Hub, on their phone or on the web, as daily check-ins and small actions. Ask for a review and their answers are back before the follow-up, beside the original.

Patient · every day

For the patient · My Lifestyle Hub

Then it becomes something they do every day.

The moment a plan goes out it lands in My Lifestyle Hub, the patient’s own app on the web, iPhone and Android. Scroll, and walk through it.

  1. My Lifestyle Hub’s 3D room: the patient’s twin, racket in hand, on its pad among the six pillar orbs, with their plans on the wall

    01 · Your room

A room of their own, in 3D.

Their twin on its pad with the six pillar orbs, their plans on the wall to open and check in on, tools to walk up to, research as books and medals in a trophy cabinet. One tap back to the classic view.

  2. Today’s check-in in My Lifestyle Hub: how you are today, then a question for each pillar

    02 · Today’s check-in

    A check-in a day, one tap at a time.

How they are, then a yes-or-no question for each of the six pillars and their plans’ own check-ins. Ten or twenty seconds, and a missed day can be answered for the six before.

  3. A plan in My Lifestyle Hub: its PDFs to keep, and the patient’s goals in their own words

    03 · Their plan

Their plan, in full, in their words.

The factors it looks at with the scores they gave, the goals they set themselves, and the resources that go with it. Nothing is rewritten for them: it is the plan their clinician sent.

  4. Your twin in My Lifestyle Hub: the patient’s own avatar on its pad, with numbered points linking their plans to the body

04 · Their twin and the six pillars

Their habits, on a body that looks like them.

A 3D twin shaped by their height, weight, age and sex, with each plan linked to the body and a page for each pillar. It is never graded, and never compared with anyone.

  5. Tools for every day in My Lifestyle Hub: the Breathing Circle and the Brain Speed Challenge

    05 · Tools

Fifteen tools that go with the plans.

The Breathing Circle, the Brain Speed Challenge, Sleep Well, the Exercise Planner and more open inside the app, and each plan brings the ones that suit it.

  6. The trophy cabinet in My Lifestyle Hub, with medals for check-ins kept up

    06 · Medals

A medal for turning up, not for a score.

Each check-in they keep up earns its own: bronze at five days running, silver at ten, gold at twenty. You see them at review, beside how often they managed each one.

Not a mock-up — this is the working dashboard, with sample patients standing in for real ones.

Custom plans

One plan for the patient with more than one condition.

Choose up to four conditions, or single factors from anywhere in the library. The platform builds one plan of exactly ten factors, and asks the factors they share once. Give it a name: that is the name the patient sees on their plan and in My Lifestyle Hub. Send it to one patient, or keep it as a template for the whole practice. On every paid tier.

For example

HypertensionCOPDWeight Loss
Heart, lungs & weightOne plan · ten factors · shared factors asked onceA practice template, ready to send again

If any of it comes from a plan for a long-term condition, a custom plan waits for a clinician’s approval, like any condition plan.

NewDrafted with AI

Describe the patient. Check the draft.

Describe a patient by talking or typing, for as long as you like: their conditions, their habits, who supports them and the help you want for them. An AI drafts a custom plan for you to check: a name and goals, the ten factors with the scores it can estimate, and check-ins and resources chosen from your own library. On every paid tier, with the 14-day free trial.

A made-up example: what the AI is sent

Margaret Hillname, 6460–69, from Perthplace, has high blood pressure and COPD. Walks her dog most mornings, sleeps badly since she retired, and wants help to stop smoking.

  1. Identifiers come out firstNames, dates, numbers, addresses and places are taken out before anything is sent, and ages become ten-year bands. You see exactly what the AI will see, and can take out anything else before it goes.
  2. Medicines are left to youAnything it writes about medicines, doses, targets or urgent symptoms is left out of the draft and listed for you.
  3. You check every lineEach score says where it came from, and what the AI wrote itself is marked. Edit anything before it goes.
  4. The patient confirmsThey answer only the questions you left, see what you estimated, and can change it. It takes them about two minutes.

Dictation works in Chrome and stays on your computer: nothing is sent while you speak. Your description and the AI’s draft are not stored. What is kept is the plan you send, and a record that a draft was made.

The evidence

Every figure checked against its paper.

One hundred and eighty-seven papers sit behind the plans. Forty carry a headline figure read back to the source — no rounded-up claims, no number without a citation. They are in the platform too: one finding a week on your dashboard, and the whole table a click away.

Why practices choose it

Built around the minute you actually have.

Approve in under a minute

Goals, self-scores, clinical measures and any flag on a single screen, with the consultation note written and ready to paste into the record.

Every patient, not just the ones who fit

The assessment happens outside the appointment, so a whole recall list can be invited at once. Capacity stops being the thing that decides who gets a plan.

Reviews that arrive before the appointment

One click sends the patient their review questionnaire. Their new answers come back beside the originals, with the movement in every factor already worked out.

An evidence base you can stand behind

Every plan is built from the research table, and every figure the platform shows links to its paper. Defensible in a clinical review, and in front of a commissioner.

MBS care plans, prepared for you

GP Chronic Condition Management Plans come out of the same assessment, with item numbers and eligibility checked against the current ruleset and kept as a versioned record.See how it works ↓

Something to show for the work

Every plan carries the factors assessed, the scores the patient gave and the goals they set — a record of structured lifestyle and preventive care, per patient and across the list.

Nothing to install, nothing to share

It runs in the browser and signs you in with a code to your own email, so an approval in the dashboard carries a clinician's name. Designed for UK GDPR Article 9.Designed for the Australian Privacy Principles.

Pricing

Per clinician, never per patient.

All prices exclude VAT, which is added at checkout. You pay per clinician seat, monthly or yearly (paying yearly saves about 9%). A 14-day free trial runs on a practice's first subscription. Patients are never charged, on any tier.
