For patients · My Lifestyle Hub

Your plan, your check-ins and your twin, in one place.

My Lifestyle Hub is the app that goes with GeneralPractice.AI. The plans your practice sends you arrive in it by themselves, and you can make lifestyle and prevention plans of your own. Each day you check in, one tap at a time, and your twin shows your habits on a body that looks like you.

It’s free, and there’s no card to give. On the web you sign in with a code sent to your email, so there’s no password to remember. Use it on the web or on your phone. It’s for adults in the UKin Australia.

Inside it

Everything for your plans, a tap apart.

Home starts with today’s check-in and your twin. From there: your plans, the library of plans to explore, the apps, help where you live, and your account.

Your plans

From your practice, or made by you.

  1. From your practiceWhen your practice sends you a plan, it appears here by itself, matched to your email address. A plan for a long-term condition appears once your clinician has approved it.
  2. Made by youAny of the 19 lifestyle and prevention plans, from Explore plans. You answer its questions, which takes about ten minutes, and the plan is yours.
  3. For a conditionThe 38 plans for long-term conditions, such as type 2 diabetes or high blood pressure, come from your GP or practice. Explore shows what each one looks at, with a note you can print to take to them.
  4. Yours to keepEach plan shows the habits it looks at with your scores, your goals and its check-ins, with resources and research. Every plan has a full PDF and a summary PDF, and goes to the phone app too.

Each day

Checking in, one tap at a time.

Today’s check-in comes first on Home. Tap an answer again to clear it, and if you miss a day, you can answer for the six days before.

  • How you areFive faces, from low to great.
  • Your six pillarsOne yes-or-no question each for move, eat, sleep, mind, substances and social. You choose the questions.
  • Your plansYour plans’ own check-ins, answered yes, partly or no. A question that two plans share is asked once.
  • More about todayAn optional diary of energy, sleep, stress and pain. “A diary for you: never shared, and nothing is worked out from it.”

Your twin

Your habits, on a body that looks like you.

A 3D figure shaped by your height, weight, age and sex, and dressed as you choose. Lines link your plans to the body, and each of the six pillars has a page of your own numbers, with where and when each one came from. Nothing on it is graded, and you’re never compared with anyone: it’s a picture to help you along, not a medical one.

On your phone

The same plans, in your pocket.

My Lifestyle Hub is on the App Store and Google Play too, for your daily check-ins, medals and reviews there. Every plan here also goes to the phone app, so you can check in on whichever suits you. Check-ins stay where you make them.

The phone app has its own sign-in. Use the same email address there, so your plans find you. Signing in with Apple? Choose Share My Email: Hide My Email gives the app a different address, and your plans won’t appear.

Questions

What people ask first.

QDoes it cost anything?+
No. My Lifestyle Hub is free, on the web and on your phone, and there’s no card to give. On the web there’s no password either: you sign in with a code sent to your email.
QDo I need a practice to use it?+
No. If you’re 18 or over, you can sign up on the web yourself and make any of the lifestyle and prevention plans. If your practice uses GeneralPractice.AI, sign up with the email address it has for you, and the plans it sends you appear by themselves.
QWho sees my check-ins?+
You do. Your check-ins, mood and diary and your twin are shown only to you, in the app. Your practice sees the plans it made for you, as it always has.
QWill it tell me what’s wrong?+
No. It doesn’t diagnose conditions, assess your risk of disease, decide your treatment or monitor any medical condition, and it isn’t monitored. For anything about your health, contact your GP practice.
QCan I use the phone app as well?+
Yes. Sign in to the phone app with the same email address, and the plans here go there too. Check in on whichever you prefer: check-ins stay where you make them.
QHow do I leave?+
In Account, choose Delete my account. The privacy notice lists what goes straight away and what stays, such as the plans your practice made, which are part of your record there.
