Who it is for · NHS & primary careGeneral practice

Chronic disease care that survives contact with a full clinic day.

The patients who most need a structured plan are the ones the appointment never has room for. GeneralPractice.AI moves the assessment out of the room: invite one patient or a whole recall list, the answers come back as a personalised, evidence-based plan, and your part is the clinical judgement — read it, edit anything, approve it in under a minute, with the note for the record ready to paste.

It fits the work you already do. The library covers the conditions your recalls already target — hypertension, diabetes, COPD, asthma, heart failure — and every approved plan records the factors assessed, the scores the patient gave and the clinician who signed it off, so there is something to show for the work.

It fits the work you already do — and in Australia the same answers prepare a GP Chronic Condition Management Plan, with item numbers, fees and intervals checked against the current MBS ruleset. See how the GPCCMP works →

How it runs

One recall list is the whole pilot.

  1. Pick one listStart with the recall you know best — diabetes, hypertension, COPD — and invite everyone on it at once, or begin with a single patient in the room.
  2. They answer at homeAbout twenty minutes, in their own words, with the instruments the condition calls for. The ones who need longer can take it.
  3. You approve each planUnder a minute per patient: self-scores, goals and flags on one screen. Long-term-condition plans always wait for your sign-off.
  4. Reviews come to youOne click sends the review questionnaire; the answers are back beside the originals before the follow-up, movement already worked out.

From the library

The plans you would lean on.

The long-term-condition plans map onto the lists a practice already runs. Every one assesses ten modifiable factors the patient scores themselves, and every one opens on the library page with the reasons each factor earns its place.

All fifty-five are on the plan library, laid out on the body they help.

Asked before choosing

The questions this page gets asked.

QDoes it add time to the appointment?+
No — that is the point of it. The assessment happens in the patient’s own time, and approval is one screen: their scores, their goals, any flag, and a consultation note ready to paste into the record. If you prefer to start in the room, generate a link and do it together.
QWho approves the plans?+
A clinician, every time. Long-term-condition plans wait for sign-off and you can edit any part before it goes out. GeneralPractice.AI is not a medical device — it supports your clinical judgement; it does not replace it.
QWhat does it cost a practice?+
The free tier covers the ten lifestyle plans with no card. Paid tiers are priced per clinician per month — never per patient, and patients are never charged for My Lifestyle Hub. The full tiers are on the pricing section of the home page.
QWhat happens to patient data?+
The platform runs in the browser and signs clinicians in with a code to their own email, so every approval carries a name. It is designed for UK GDPR Article 9 and the Australian Privacy Principles — the detail is in the privacy policy.
